If your insurance covers surgical weight loss below are some testing and documents that you should start completing.

• Chest X-ray
• Labs (CMP, CBC, Iron and TIBC, TSH, Fasting Lipid Panel, A1C, Vitamin D, 25 Hydroxy, Vitamin B12/folate, Vitamin B1)
• EKG
• Primary Care Physicians Letter (sample letter under patient documents)
• Attend Online Support Group – 3^rd Monday of each month at 6:00 pm – https://zoom.us/join; Meeting ID 873-0912-3691, Passcode 359829
• New Patient Paperwork – complete and email to the office at mmcgee@surgicalpartnersok.com
